METHOD 1 (call us and we will complete your registration over the phone):

• Please have all the information required easily accessible to speed the process

METHOD 2 (email):

• Left click your mouse at beginning of word 'registration form' and, while holding down left button, drag mouse to bottom right (everything will highlight) and release left click.
• Press 'control' key, and while holding it, press 'c' on your keyboard (this will 'copy' the highlighted information)
• Open a blank 'Word document' and left click within the document.
• Press 'control' key, and while holding it, press 'v' on your keyoard (this will 'paste' the information you highlighted previously)
• Fill in the appropriate information on the Registration Form
• Save a copy in your documents for your records.
• Email us at the appropriate location and attach the document you just saved.

METHOD 3 (fax):

• Follow method 1 above to save a copy in your documents for your records.
• Print a copy of the Registration Form.
• Fax or mail to appropriate Academy location.

Your enrolment is accepted upon submission of this Registration Form along with payment by Cheque, Cash, Money Order, Visa or Master Card.  

To enrol by fax or mail, please print and mail this form with payment to the above address or fax to Academy of Holistic Modalities inc 519-745-4551.  You may enrol by phone with Visa or MasterCard; call 519-745-5971. Fax or bring your form with you to class. 

We encourage you to register early as the courses and workshops are filled on a first-come, first served basis.

You will be notified of cancellation 5 days prior, should a class not meet the minimum registrations. Should a course/workshop be cancelled due to lack of enrolment, your money will be refunded in full.   

Cancellations received in writing will be accepted up to two weeks prior to the date of the course or workshop, less an administration fee of $25.00. Cancellations received after this time will not be refunded, however, transferring paid fees towards another course or workshop is acceptable. There are no refunds for no-shows.

Pre-registration guarantees you a seat.

The EnerCHI^© Heated Stone Massage Program requires a minimum deposit of $350. Balance of tuition plus GST to be paid at first class.
